Trigeminal neuropathy is characterized by paroxysmal severe pain, precipitated by innocuous stimuli, and unpredictable remission and recurrence. We report a case of dentigerous cyst masquerading as recurrent trigeminal neuralgia. A 31-year-old woman developed paroxysmal and persistent neuralgic pain in her left cheek, involving the trigeminal maxillary branch. She had experienced previous attack a year ago with similar pain at same area. After admission, the only initial laboratory finding was elevated C-reactive protein level. Brain MRI showed no compressive lesion around trigeminal nerve. Facial CT showed a periodontal abscess at the left lateral maxillar incisor. With reanalysis of brain MRI, we found enhancing lesion in oral cavity reflecting periodontal abscess at sagittal thin-section view. Pain improved with excision of the abscess. Therefore, meticulous brain and facial imaging for differential diagnosis of secondary causes including dental problems should be considered, especially in young patients with increased inflammation and persistent facial pain.
